About Jordan

Jordan is a registered nurse and community care navigator at Laurelwell's Family & Community Center. He helps patients work through the practical steps that can make care difficult to reach, from understanding a referral to coordinating follow-up and connecting with transportation and community resources.

He is not a primary-care provider, and he does not replace one. He works alongside them, so a patient's care team knows who is handling the next step and the patient does not have to carry the whole plan alone. For Jordan, getting there and getting help are part of care.
